LDL/HDL Ratio: When Numerator and Denominator Move Together

The LDL/HDL ratio is your LDL cholesterol divided by your HDL cholesterol, and a result below 3.0 is generally considered acceptable. For men this is the number that swings most violently on anabolic androgenic agents, because both halves of the fraction move the wrong way at once: the numerator climbs while the denominator sinks.

Those two movements do not add up, they compound. From 3.0 divided by 1.30 to 4.2 divided by 0.60 the result jumps from 2.3 to 7.0: more than three times as high, while neither value doubles.

Do not expect much more of it than that: no guideline sets a target for this ratio. Its value lies in your own trend line, not in a cut-off.

No established reference range

There is no generally accepted Dutch reference value for the LDL/HDL ratio. The NVKC does publish a cholesterol ratio - total cholesterol divided by HDL (below 5, ideally below 3.5) - but no LDL/HDL ratio. The NHG-Standaard CVRM (2024) no longer uses a cholesterol ratio for risk assessment, looking instead at LDL- and non-HDL-cholesterol themselves. For the LDL/HDL ratio specifically there is therefore no Dutch standard we can show; we would rather state no number than one without a source. Discuss your cholesterol with your doctor, looking at your LDL, your non-HDL and, if useful, the cholesterol ratio (total/HDL).

LDL/HDL Ratio: what this test measures

The LDL/HDL ratio is a fraction. The numerator holds your LDL cholesterol, the denominator your HDL cholesterol. The laboratory determines those two, divides one by the other and prints the result. No extra tube of blood goes in. What you are measuring here is not a new substance but the relation between two numbers already sitting elsewhere on the same form.

It is precisely that fractional shape which makes the number so mobile. Dividing by a denominator below one is the same as multiplying: at an HDL of 0.60 mmol/l your LDL is multiplied by more than one and a half. If the denominator sinks while the numerator climbs, those movements do not cancel out, they stack.

The worked example below shows how far that goes. Not one individual value triples. The result does.

In the bottom row the LDL has risen by four tenths of a mmol/l and the HDL has more than halved. Looked at separately, both numbers shift a little way and no further. Combined in one fraction they stand at more than three times the starting value. That is not alarmism but arithmetic, and it is the one spot on your report where a shift in two values at once becomes visible in a single figure.

Two things to hold on to alongside it. The LDL in the numerator is, in most laboratories, itself already a calculated number derived from the triglycerides, which makes the whole fraction sensitive to the conditions of the draw. And this is not the same figure as the cholesterol/HDL ratio: that one puts total cholesterol in the numerator and therefore sits on a scale around 5 rather than around 3. Anyone who mixes the two up misreads every result they ever get.

LDL/HDL Ratio: why this value matters

Start with what this number is not. No guideline sets a target for it. The European ESC/EAS guideline and the Dutch CVRM standard steer on LDL cholesterol itself, with non-HDL cholesterol and ApoB as secondary goals. The LDL/HDL ratio does not feature in either, so your doctor will not base a treatment decision on it.

Where the number does add something is as a trend line within one and the same person. Because numerator and denominator can both move, the fraction responds earlier and more strongly to a shift in your lipid metabolism than either value on its own. That makes it sensitive as an early signal: the ratio can expose a change that still sits comfortably inside the reference range in the individual figures.

The situation where this shows up most clearly is the use of anabolic androgenic steroids. Androgens push HDL down while LDL climbs, and with oral, 17-alpha-alkylated agents that HDL suppression is sharpest. Both halves of the fraction then move the wrong way at once, so the result can double or worse while each individual value merely looks somewhat off. We are describing what happens to a blood value here, not what you should or should not use: that judgement belongs with a doctor.

That sensitivity is only usable in one direction. You compare yourself against yourself, measured under matching conditions, and not against a cut-off. A good cholesterol ratio from one isolated draw says little; three measurements at the same laboratory say a great deal more, even when the number stays inside the reference range throughout.

The familiar weakness of any fraction applies here in full: the absolute amount of harmful cholesterol disappears into it. An LDL of 2.6 mmol/l with an HDL of 1.0 mmol/l gives 2.6. An LDL of 3.9 mmol/l with an HDL of 1.5 mmol/l gives exactly the same. The second man carries half again as much atherogenic cholesterol at an identical number, and it is that absolute LDL which accumulates in the artery wall over decades.

Then the limit of 3.0 itself. It is unisex, while men's HDL runs on average some 0.2 to 0.3 mmol/l lower than women's. At the same LDL your result therefore comes out structurally higher than your partner's, with nothing behind it but your sex. The unisex upper limit is in practice stricter for men, and placing your number next to a woman's yields no usable information.

LDL/HDL Ratio: when is testing worthwhile?

You do not order this ratio on its own. It falls out of any lipid panel automatically once the LDL and the HDL have been determined. So the real questions are when you have blood drawn and what you will later compare the result against.

If you want to see a line, you first need a starting point. A single measurement cannot, by definition, rise or fall. A first determination in a settled period, followed by repeats at fixed intervals, is what turns this number into a trend. Choose the same laboratory every time, otherwise you are comparing two methods rather than two moments.

Fasting weighs more heavily with this fraction than with most lipid values. Total cholesterol and HDL barely change after a meal, but triglycerides do rise, and your LDL is calculated from them. Above roughly 4.5 mmol/l of triglycerides that calculation stops being valid and the ratio says nothing whatsoever.

Watch the natural variation before you place two results side by side. Your LDL and total cholesterol fluctuate within you by five to ten percent from day to day, the triglycerides by twenty to twenty-five percent, and that noise feeds straight into the fraction. Wait a number of weeks, too, after a substantial infection, surgery or a heart attack, because cholesterol dips temporarily in that period and you would underestimate your usual value.

If your result is new and markedly abnormal, have an underlying cause ruled out before lifestyle or anything else takes the blame. Think first of an underactive thyroid: it raises LDL, it is common and it is easily overlooked, so let a TSH be part of the workup. Poorly controlled diabetes, kidney disease, cholestasis, liver disease, heavy alcohol use and a range of medicines can equally explain the picture. Discuss every result with a doctor, together with your individual values and your full risk profile.

LDL/HDL Ratio: symptoms of a high or low value

Low Levels

You will not notice a low LDL/HDL ratio. Cholesterol gives no signals, whatever proportion it circulates in, so symptoms of a low value simply do not exist. In itself a low result is no problem either: the denominator is then large relative to the numerator.

With a fraction it is still worth asking which half produced that low result. A ratio of 2.0 at an LDL of 4.0 and an HDL of 2.0 is something entirely different from a ratio of 2.0 at an LDL of 2.0 and an HDL of 1.0. The first man carries twice as much LDL around with him while the number is identical. A favourable proportion therefore says nothing about the quantity.

On top of that, the result can come out low for technical reasons. With high triglycerides or a draw that was not fasted, the calculated LDL comes out too low and the fraction looks better than it is. So always look at the individual values behind it, and have a doctor assess the whole picture.

High Levels

You will not feel a high LDL/HDL ratio either. Atherosclerosis progresses silently for years: there is no fatigue, no complaint about libido and no dip in the gym from which you could read off an unfavourable proportion. What you do notice belongs to the cause underneath, not to the number.

So always ask which half has moved. A high result can come from a risen LDL, from a fallen HDL, or from both at once, and that third variant climbs by far the fastest. In men, a low HDL, high triglycerides and reduced insulin sensitivity are a common combination. Smoking lowers HDL as well, and anabolic androgenic agents push HDL down substantially while LDL climbs.

Meanwhile, do not skip the causes that have nothing to do with lifestyle or substances: an underactive thyroid, poorly controlled diabetes, kidney or liver disease, cholestasis and a number of medicines. A high ratio is not a diagnosis and not proof of cardiovascular disease. It is a reason to pull up the individual values and to go through your full risk profile with a doctor.

LDL/HDL Ratio: lifestyle and this value

The first thing to do with this number is not to chase it. A fraction has two dials, and turning the denominator changes nothing about the amount of cholesterol that can accumulate in your artery wall. Research into agents that raised HDL and nothing else came unstuck on exactly that: the denominator grew, the number of cardiovascular events did not shrink.

What genuinely brings LDL down is well established and thoroughly unexciting. Replace saturated and trans fat with unsaturated fat, get fibre every day from pulses, wholegrains and vegetables, lose surplus abdominal fat, keep moving and do not smoke. Stopping smoking also lifts HDL, which is one of the few steps where the fraction and your health travel in the same direction. Alcohol does not belong on this list: it does raise HDL and so flatters the result, but it drives triglycerides up at the same time and does not improve your profile.

If you use anabolic androgenic agents, or are thinking about them, a blood value is not an instrument for steering yourself. The effect on HDL is large and arrives fast, and the fraction can triple while both individual values still look within reach. That is a conversation with a doctor, not a number to react to on your own.

When you place results side by side, do it under matching conditions: the same laboratory, fasted, and not shortly after illness or surgery. If your report carries an ApoB alongside this ratio, let that weigh more heavily, because it counts the number of harmful particles rather than their cargo.

And finally: a self-ordered result is never the trigger for starting, stopping or rearranging a cholesterol-lowering medicine. Your doctor makes that call, with your full risk profile in front of them.

LDL/HDL Ratio: frequently asked questions

Why does my ratio swing harder than my individual cholesterol values?

Because it is a fraction and both halves can move at once. If your LDL rises from 3.0 to 4.2 mmol/l while your HDL sinks from 1.30 to 0.60, the result goes from 2.3 to 7.0. The LDL is then forty percent higher and the HDL more than halved, yet the ratio stands at over three times its starting value. Dividing by a smaller number multiplies the effect.

Can my ratio double while my LDL barely changes?

Yes. If the HDL alone halves, say from 1.20 to 0.60 mmol/l, the result doubles at exactly the same LDL. It works the other way round too: with a stable HDL the ratio rises in proportion to the LDL. So always check which half of the fraction moved before you attach any conclusion to the change.

Does the upper limit of 3.0 apply to men as well?

It is the same limit for both sexes, but men have on average an HDL some 0.2 to 0.3 mmol/l lower. At the same LDL your result therefore comes out structurally higher than a woman's. The unisex limit is in practice stricter for men, and comparing your number with your partner's gains you very little.

What is a good cholesterol ratio for tracking my progress?

Track it as a line for yourself, not as a score against a cut-off. Below 3.0 counts as acceptable, but that is a population reference interval and not a treatment goal: no guideline sets a target for it. Three measurements under matching conditions tell you more than one isolated result landing just under or just over the limit.

How big does a change have to be before it means anything?

Bigger than the noise. Your LDL and total cholesterol naturally fluctuate by five to ten percent, the triglycerides by twenty to twenty-five percent, and that variation feeds into the fraction. A ratio moving from 2.4 to 2.6 therefore says little. A doubling sits well outside that margin and does deserve a conversation with your doctor.

My total cholesterol stayed flat but my ratio went up. How?

Total cholesterol is roughly the sum of your LDL, HDL and VLDL. If the LDL rises by half a point while the HDL falls by about the same, that sum stays almost unchanged and you never see the shift in the total. The fraction does show it, because numerator and denominator move in opposite directions there. So always compare the individual values.
