LDL Cholesterol in Men: Why a Normal Value Can Say Too Little

LDL cholesterol measures how much cholesterol sits inside your LDL particles. It does not measure how many particles that is, and that distinction is exactly why a normal LDL in men carrying central fat is often more reassuring than it should be.

Under insulin resistance, LDL particles become smaller and denser. It then takes more than one particle to carry the same amount of cholesterol, so at an identical LDL value more particles pass through your artery wall. You only see that difference if you also measure apolipoprotein B. LDL Cholesterol: what this test measures

LDL cholesterol is a measure of cargo, not of count. The lab totals how much cholesterol sits inside all your LDL particles and prints one number underneath. How many particles carry that cargo stays out of view.

That sounds like a detail, but it is the most important limitation of this blood value. Artery narrowing is driven by the number of particles entering the artery wall, not by the amount of cholesterol they happen to be carrying. Every atherogenic particle carries exactly one apolipoprotein B, so that value is a direct count.

In men with central fat and early insulin resistance, the two numbers drift apart systematically. The particles become smaller and denser, and smaller particles carry less cholesterol each. So more are needed, and the count climbs while the LDL value stays put. These are worked examples showing the pattern.

The recognisable clue that you are in the second row is already on your result: raised triglycerides together with a low HDL. That combination belongs to insulin resistance, and it is exactly there that LDL diverges most often from the real particle burden.

There is one more thing: in a routine lipid profile the LDL is calculated from your triglycerides. If those are raised, that calculation underestimates the LDL on top of everything else. Two sources of error in the same direction, in the same group of men.

LDL Cholesterol: why this value matters

For men, the most useful question is not whether your LDL sits above the line, but whether your LDL and your particle count say the same thing.

In roughly one person in five they do not. That discordance is neither rare nor a measurement error: it is the normal consequence of a changed particle size. When the two diverge, apolipoprotein B is the better predictor of risk. That matters clinically, because the group where discordance is most common, men with central fat, raised triglycerides and a low HDL, is exactly the group being reassured by a tidy LDL.

On top of that, men develop cardiovascular disease on average about ten years earlier than women. A raised LDL at forty therefore weighs more than the number alone suggests, simply because the exposure runs for longer before you reach the age at which risk normally climbs.

Then the limit on the page. The 3.0 mmol/l is a population reference value, not a target. Guidelines tie the goal to your risk category, built from your age, your blood pressure, smoking, diabetes, your kidney function and your family history. An LDL of 2.8 is normal on paper and clearly too high for someone who has already had a heart attack.

Finally on androgens, because it comes up often in this group and is rarely explained well. Testosterone at replacement doses has a modest and not entirely consistent effect on the lipid profile: total cholesterol and LDL usually fall slightly, and HDL falls as well. That is a very different thing from oral 17-alpha-alkylated androgens, which lower HDL sharply and raise LDL clearly. If you use something from either category, say so when your lipid profile is assessed. Without that information your diet gets blamed while the explanation lies elsewhere. Never change prescribed medication yourself on the basis of a self-ordered result.

  • Water is always fine. Drink as usual beforehand, it makes the draw easier.
  • Take your usual medication, unless your own doctor told you otherwise.

LDL Cholesterol: when is testing worthwhile?

Have your lipid profile drawn in an ordinary period, not an exceptional one. A weekend of heavy drinking raises your triglycerides for days afterwards, and because your LDL is calculated from those triglycerides, your LDL shifts with them. Two to three alcohol-free days before the draw gives a fairer picture.

If you are considering starting testosterone therapy, have a full lipid profile done first. Without a baseline you cannot later establish whether a change comes from the treatment or from your lifestyle, and that distinction is exactly what you need.

Measure apolipoprotein B alongside your LDL at least once. If the two agree, LDL will do from then on. If they diverge, you know to watch the particle count rather than the cargo. The same holds for a one-off Lp(a) measurement: that value is largely genetically set, barely changes across your life, and does change how your doctor reads your LDL.

If your LDL is unexpectedly high, rule out an underlying cause before the conversation about food starts. An underactive thyroid is the classic missed explanation and is settled with a TSH test. Where insulin resistance is suspected, an HbA1c belongs in the same batch.

Hold off after surgery, a serious infection or a hospital admission: LDL runs below your habitual value for weeks afterwards.

LDL Cholesterol: symptoms of a high or low value

Low Levels

A low LDL value causes no symptoms. You notice nothing in your energy, your strength or your libido, because cholesterol is not a value you feel.

One misconception is stubborn in this group: that low cholesterol undermines your testosterone production, since cholesterol is the building block of steroid hormones. The reasoning sounds logical but does not hold up in practice. Your body makes ample cholesterol for hormone production on its own, and a low LDL in the blood does not mean your cells are short. A low testosterone almost always has a different explanation.

If you take a lipid-lowering medicine, a low LDL is the intended result, not a side effect. Never stop or change that medication on the basis of a favourable self-ordered result.

If your LDL is unexpectedly low without medication, it can fit with an overactive thyroid, liver disease, poor absorption in the gut, undernutrition or a chronic illness. Have that assessed rather than drawing your own conclusions.

High Levels

A raised LDL does not hurt and costs you no performance. You notice nothing, which is precisely why men find out later than is good for them: there is no complaint sending you to a doctor. The build-up in the artery wall is silent, over decades, and the first noticeable sign can be a heart attack.

With very high, hereditary values, visible cholesterol deposits can appear over the years: a thickened Achilles tendon, thickening in the extensor tendons on the back of the hand, yellowish patches around the eyelids or a grey-white ring around the cornea at a young age. Active men almost always attribute a thickened Achilles tendon to training load; have it assessed when your LDL is markedly raised and heart problems appeared early in your family.

Erectile difficulties deserve a separate sentence here. They are not a symptom of a high LDL, but they are a recognised early signal of vascular problems in general, because the vessels involved are small and get into trouble sooner. So discuss that complaint with a doctor rather than treating it as unrelated to your cardiovascular risk.

Without exception: chest pain or pressure, or sudden severe breathlessness, means calling the emergency number. Never a blood test.

LDL Cholesterol: lifestyle and this value

Start with the cause. If your LDL has risen unexpectedly, have your thyroid checked before you start adjusting your diet.

If you sit in the pattern of raised triglycerides with a low HDL, the lever is not primarily your LDL but your visceral fat. The fat around your organs drives the insulin resistance that makes your particles small and numerous. Weight loss lowers your triglycerides substantially and raises your HDL, and so lowers your particle count while the LDL itself moves only modestly. Which is exactly why apolipoprotein B is the better measure to read your progress from.

Alcohol deserves separate attention in this pattern: it raises triglycerides directly and is, in many men, the single largest contribution to an unfavourable lipid profile. Cutting back often achieves more here than any adjustment to the fat content of the diet.

For the LDL itself, the type of fat remains decisive: replace saturated fat from fatty meat, butter and full-fat dairy with unsaturated fat from olive oil, nuts, seeds and oily fish. Soluble fibre from oats, barley and pulses lowers LDL measurably as well.

Sleep belongs in this list. Sleep apnoea is common in men carrying excess weight and goes together with an unfavourable lipid profile, mainly higher triglycerides and a lower HDL. If you snore heavily and are sleepy during the day, raise it with your doctor; it is one of the few things on this page where treatment can shift the whole picture.

My LDL is normal but my apolipoprotein B is raised. What does that mean?

That your cholesterol is spread across more, and smaller, particles than average. The cargo is normal, the particle count is not. Because particle count drives artery narrowing, apolipoprotein B is the better measure in that case. This pattern belongs to insulin resistance and often occurs together with raised triglycerides and a low HDL. Discuss it with your doctor.

Why should I measure apolipoprotein B alongside my LDL?

Because in roughly one person in five the two do not say the same thing, and you cannot know which group you are in until you measure. If they agree, LDL will do from then on and you only had to do it once. If they diverge, you know your LDL has been presenting your risk as structurally better than it is.

Does testosterone therapy raise my LDL cholesterol?

Possibly, and that is the point of this page. Central fat drives insulin resistance, which makes your LDL particles smaller and more numerous without the LDL value changing. So look at your triglycerides and your HDL on the same result, and measure apolipoprotein B once to be sure.

What does alcohol do to my LDL value?

Alcohol mainly raises your triglycerides, and because your LDL is calculated from those triglycerides, your LDL result shifts with them without your actual particle burden necessarily having changed. With regular use the whole profile does genuinely deteriorate. Leave two to three alcohol-free days before a test you intend to take seriously.

My LDL is raised and I am only forty. Is that early?

It is an age at which it is worth acting, because exposure adds up over the years and men develop cardiovascular disease on average about ten years earlier than women. A markedly raised value without medication, especially with early heart problems in the family, is a reason for a doctor to look further.
