CK value (creatine kinase) too high: from sport or something else?

A high CK value after sport is often just muscle damage from your training. Creatine kinase (CK) is an enzyme that leaks from your muscles when fibres are damaged. After a heavy or unfamiliar workout, CK can rise to a few times your normal value and stay up for a few days. Usually it falls again on its own.

Still, not every high CK is harmless. It's about context: what did you do, how high is the value, and do you have symptoms?

What is a high CK value exactly?

CK sits mainly in your muscles. When muscle fibres are damaged, CK is released into your blood and the value rises. So a raised CK usually means there's muscle damage somewhere, from whatever cause. On its own it doesn't say what that cause is.

That makes CK a good marker to track, but a poor one to interpret alone. One high number without a story says little.

Is my high CK from sport?

If you trained hard or unusually shortly before the test, a raised CK is easy to explain. Eccentric load in particular, like the lowering phase of squats or lots of downhill running, causes muscle damage and a CK peak. In trained people the values also often sit higher than the standard reference range (Mougios, 2007).

The peak usually comes 24 to 72 hours after exertion and then drops. If you test right after your heaviest session, you measure that peak and not your resting value.

When does a high CK not fit sport?

Sometimes a high CK comes from elsewhere. Cholesterol-lowering drugs (statins) can cause muscle complaints and a raised CK, even without training. A viral infection, certain muscle conditions or heavy alcohol use can also play a role. The table below helps tell them apart.

A separate CK-MB test is used to check whether part of the CK comes from the heart muscle rather than your skeletal muscles.

What is a normal CK value?

Reference values for CK differ per lab and are usually given in units per litre. More important than the exact figure is your situation: if you train a lot, your normal value can sit higher than in someone who doesn't. A lab compares your result with a general reference, not an athlete's value.

Don't confuse CK with LDH, another enzyme that can rise after exertion. More on that in LDH too high.

When do you see your GP?

Don't wait with a very high CK plus dark, cola-coloured urine, severe muscle pain or muscle weakness. That can point to rhabdomyolysis, a serious form of muscle breakdown. Also discuss muscle pain while using statins with your GP.

For an ordinary, mildly raised CK after a heavy workout, rest is usually enough. Discuss a result you don't trust with your GP, who can put your value in context.

What can you do yourself about a raised CK?

If your CK is raised after a heavy workout and you otherwise feel fine, rest is often the main thing. Give your muscles a few days to recover before training hard again. Drink normally and build up your load calmly, especially if you're trying something new in the gym.

If you want to repeat the test, do so after a few easy days rather than right after your heaviest session. That way you see your resting value more clearly. If the CK stays high without a clear reason, or symptoms such as muscle weakness appear, discuss it with your GP.

Don't experiment on your own with stopping or changing medicines. If you think a statin is contributing to your muscle complaints, put that to your doctor. The pattern matters most here: a one-off peak after exertion is different from a value that stays high for no reason.
