Complete Men's Health Panel
A broad men's health panel: hormones, heart, metabolic, and organ function in one blood draw.
Troponin is a protein in the heart muscle that lets the muscle cells contract and normally stays inside those cells. With a heart attack or other damage to the heart muscle, cells perish and the protein then leaks into the blood.
Above the laboratory limit, troponin T can point to heart muscle damage; besides a heart attack, the Dutch GP guideline names causes outside the heart, such as end-stage kidney failure. The level of troponin T only shows in a blood test.
Star-SHL reads troponin T in three zones: below 14 ng/l (nanograms per litre) negative, 14 to 50 ng/l borderline and above 50 ng/l positive. In the borderline zone, Star-SHL looks at the difference with a second measurement within 3 hours if in doubt.
Doctor's Assessment Included
Troponin T is a heart muscle protein that enters the blood when heart muscle cells die. Star-SHL and Unilabs use a limit of 14 ng/l, Certe the same limit from age 18.
| Result | Value (ng/l) |
|---|---|
| Normal | < 14 |
| Borderline | 14–100 |
| Markedly elevated | ≥ 100 |
These cut-off values apply to the high-sensitivity troponin T test (hs-cTnT, Roche Elecsys). 14 ng/l is the 99th percentile of a healthy population; Dutch laboratories apply this limit to men and women alike. They interpret a value below 14 ng/l as no sign of heart muscle damage, 14 to 100 ng/l as equivocal, and above 100 ng/l as consistent with heart muscle damage. Important: a normal troponin value does not rule out a heart attack, and a raised value does not prove one. Troponin is a test for acute diagnosis when there are symptoms (such as chest pain), assessed by a doctor together with the course of the symptoms and an ECG. It is not a screening test for healthy people, and it can never be judged on a single value (in a heart attack, troponin rises and falls over several hours; the diagnosis requires repeated measurements). A raised value can also fit kidney dysfunction, heart failure, pulmonary embolism, sepsis or heavy exertion. With a raised result or acute symptoms: contact a doctor immediately or call 112. The literature also describes sex-specific limits (lower for women, higher for men); Dutch laboratories report against the shared limit of 14 ng/l.

At Star-SHL, Certe and Unilabs, the amount of troponin T in the blood is given in ng/l (nanograms per litre), with 14 ng/l as the published limit; Certe applies that limit from age 18. Star-SHL (serum) and Certe (plasma) measure with a high-sensitive test, and Unilabs has troponin T determined in plasma via VUmc.
The NVKC, the Dutch society for clinical chemistry, describes two measurable forms of troponin, troponin I and troponin T, and writes that the normal value depends on the test used; the limits for troponin I and troponin T are not interchangeable.
Troponin T is the blood test for heart muscle damage, and the Dutch GP guideline uses an acute rise or fall in it in the definition of a type II heart attack. In that type, according to the guideline, there is also an imbalance between the oxygen the heart needs and the oxygen it gets, unrelated to acute atherothrombosis in a coronary artery, plus at least one of the other criteria the guideline names, such as new ischaemic changes on the heart tracing.
An international review from 2021 links a higher troponin, acute or long-lasting, to a lower chance of staying free of a serious medical event, in patients and in the general population.
Pause biotin
RequiredDo you take a supplement with biotin (vitamin B8, common in hair, skin and nail supplements)? Stop it 72 hours before the draw. Biotin interferes with the measurement of these values.
No hard training beforehand
RequiredNo heavy training, long endurance sessions or anything that leaves you out of breath in the 48 hours before the draw. Normal walking is fine.
The NVKC, the Dutch society for clinical chemistry, names chest pain as a complaint for which the doctor can have a troponin test done, to work out whether there may have been a heart attack. Pain or heavy pressure on the chest that does not ease after sitting down is, according to Thuisarts (the Dutch GP information site), a reason to call 112 straight away.
When chest pain or pressure is not the main complaint or is absent, the Dutch GP guideline advises the GP to consider weighing risk factors in the decision to send someone on to hospital. As such factors, the guideline names older age, smoking, diabetes and chronic kidney damage, with which the prior likelihood of an acute coronary syndrome is higher.
Star-SHL reads a troponin T below 14 ng/l as negative, and Unilabs and Certe (from age 18) use the same limit. A low value causes no symptoms of its own and, according to the NVKC, fits a healthy heart.
Above 14 ng/l (Star-SHL, Unilabs, Certe from age 18), troponin T gives no sensation of its own; when a heart attack is the cause, its chest pain can feel pressing according to Thuisarts, the Dutch GP information site, with sweating, nausea, shortness of breath or dizziness often alongside. If the pain or a heavy pressing feeling on the chest does not go away once someone sits down, Thuisarts advises calling 112 straight away.
Between 14 and 50 ng/l, Star-SHL advises a repeat test within 3 hours if in doubt. Above 50 ng/l, Star-SHL reads the result as positive. According to the Dutch GP guideline, end-stage kidney failure, a pulmonary embolism, sepsis and a stroke raise troponin too; with these conditions there is heart damage, but according to the guideline it does not stem from a shortage of oxygen to the heart muscle.
Heart attack
In a heart attack, heart muscle cells die and troponin is released; according to the NVKC it is measurable in the blood within 3 to 4 hours and stays raised for 1 to 2 weeks.
Pulmonary embolism
According to the Dutch GP guideline, a pulmonary embolism can raise troponin: there is damage to the heart, but no lack of oxygen to the heart muscle as the underlying cause.
Sepsis
Sepsis is among the causes of a troponin rise outside the heart in the Dutch GP guideline, with damage to the heart without a lack of oxygen to the heart muscle as the underlying cause.
Stroke
A stroke is among the causes of a troponin rise outside the heart in the Dutch GP guideline, with damage to the heart without a lack of oxygen to the heart muscle as the underlying cause.
End-stage kidney failure
End-stage kidney failure is among the causes of a troponin rise outside the heart in the Dutch GP guideline.
With a raised troponin T, a doctor looks for the cause, and the treatment follows that cause. For pain on or in the chest that keeps coming back without a known cause, Thuisarts advises making an appointment with the GP on a working day; for pain or heavy pressure that stays at rest, 112 is the step.
Star-SHL reads a troponin T between 14 and 50 ng/l as borderline. If in doubt, Star-SHL advises a repeat test within 3 hours; a difference of more than 7 ng/l between the two measurements then fits an acute coronary syndrome.
Yes, end-stage kidney failure is among the causes of a troponin rise outside the heart in the Dutch GP guideline on acute coronary syndrome. With such a rise, according to the guideline, there is damage to the heart, but a lack of oxygen to the heart muscle is not the underlying cause.
For the troponin T result, the planning at Caliberhealth allows 96 hours after the blood draw, and before that we arrange a ZorgDomein referral that takes one business day. With pain or heavy pressure on the chest that stays at rest, Thuisarts advises calling 112 straight away.
