Tired despite enough sleep: what could be going on?

You slept eight hours, your body lay horizontal, the alarm did not go off in the night. And still you drag yourself through the morning and reach for coffee and sugar around three. That is frustrating, because on paper you are doing everything right. The bed checks out, the feeling does not.

We often see this pattern in men with a busy job, and the first conclusion is almost never laziness. Often something physical or hormonal is at play, and that is exactly where you can do something once you know what it is.

Why are you tired while you sleep enough?

Enough hours in bed is not the same as good sleep. Restless sleep, waking often or snoring with pauses can undermine your rest from the inside, even if your sleep duration looks fine on paper. The Dutch Brain Foundation (Hersenstichting) points out that sleep quality is at least as important as sleep duration for how rested you feel.

In addition, deficiencies and hormones can play a role, separate from your sleep. A low ferritin, an underactive thyroid or a low total testosterone cause energy complaints that have nothing to do with your nights rest.

The tricky thing about this kind of fatigue is that you can explain it away for a long time. Pressure at work, a new baby, the winter: there is always some explanation that stops you from digging deeper. But if the fatigue lasts for weeks and your life does not look clearly different from a year ago, that is exactly a reason to look further.

What role do stress and cortisol play?

Long-term stress keeps your body in high gear, even at night. Your cortisol can then become disrupted, making your sleep shallower and leaving you tired during the day anyway. The annoying part is that this sustains itself: tired because you are tense, tense because you are tired. You can read more in our pillar always tired as a man.

What does your lifestyle say?

Before you think of deficiencies and hormones, it is worth looking at the basics. The Dutch public health institute RIVM names enough exercise, healthy nutrition, little alcohol and good sleep as pillars of a healthy lifestyle. A lot of lingering fatigue sits in a combination of too little movement, irregular eating and too much caffeine or alcohol at the wrong moments.

That sounds boring, but it is where much of the gain is. Only once the basics are in order and you are still tired does blood testing become really interesting.

Does your mood play a part too?

Fatigue and a low or flat mood often blend into each other. Long-term stress, little enjoyment in things and persistent tiredness can occur together, and it is not always easy to separate cause from effect. A blood test can map out physical causes, but says nothing about the mental side.

If you notice that the fatigue goes with low mood, little motivation or a short fuse, take it seriously and discuss it with your GP. On average, men go to the doctor later with this, and that helps no one. Physical and mental are connected, and one does not rule out the other.

When do you go to the GP?

If the fatigue lasts longer than a few weeks, if you fall asleep unintentionally during the day, or if you snore with pauses, discuss it with your GP. A blood test can map out the physical side in advance, so you enter the conversation more focused. See the result as a starting point, not a diagnosis, and always discuss abnormal values with a doctor.
