Healthy ageing for men depends largely on a handful of choices: moving enough, eating well, not smoking and being moderate with alcohol. Dull, I know. But the payoff is big. Men who combined five healthy habits at age fifty lived on average 12.2 years longer without chronic disease than men who had none (Li et al., 2018).
Still, there is a part of the story most men do not hear. As you age, a few things shift on their own, regardless of how well you live. Your testosterone drops slowly, you lose muscle mass faster and your blood vessels stiffen. What strikes me: men often wait until something breaks before they measure anything. That is a shame, because the starting point is exactly what counts.

What does healthy ageing for men actually mean?
Healthy ageing is not only about living longer, but about staying fit and independent for longer. Researchers call that healthy life expectancy: the years you spend without serious limitations. The goal is not to stop ageing, but to stretch your fit years.
For men, something specific plays a part. From around forty, several things start to shift at once: your hormone balance, your muscle mass and your metabolism. None of those shifts is a disaster on its own, but together they shape how you feel at sixty.
The good news is that you have influence over many of them. Not all, and not completely, but enough to make a difference. That is what the rest of this article is about.
Which lifestyle factors matter most?
Four habits surface in almost every large study: moving enough, eating well, not smoking and being moderate with alcohol. Together they determine a large part of your later health. No single habit is a miracle, but the stacking counts heavily.
Movement may be the strongest dial. In a pooled analysis of more than 660,000 adults, the biggest health benefit already sat in the first blocks of activity per week, plateauing around three to five times the basic recommendation (Arem et al., 2015). So you do not have to become a cyclist to capture most of the gain.
Food is the second pillar. The Dutch Nutrition Centre (Voedingscentrum) advises a pattern with plenty of vegetables, fruit, wholegrains, legumes and fish, and little processed meat and sugar. For men who want to keep muscle, extra attention for protein is often added.
What strikes me in the stories I hear: men underestimate how much the basics still do at a later age. Starting at sixty is late, but certainly not too late.
How important are sleep and stress?
Sleep and stress determine more of your health than many men think. Long-term stress and poor sleep are linked to a higher risk of cardiovascular disease, weight gain and disturbed blood sugar. They are not a side issue in healthy ageing.
For men, something is added. Poor sleep can lower your testosterone production, and lower testosterone makes you more tired during the day. That can create an unpleasant circle. How that sleep-hormone cycle works is in our article on poor sleep and testosterone.
Chronic stress also keeps your cortisol raised for longer, and that can affect your metabolism and sleep over time. That does not mean every busy week is harmful, but long-term overload does deserve attention.
The good news is that you have influence here. Regularity in your day, movement in the morning and less caffeine and alcohol in the evening often help. Small, sustained adjustments add up more here than one big intervention.
Does your testosterone change as you age?
Yes, usually. From around forty your testosterone falls slowly, on average about 0.1 to 0.2 nmol/l per year (Harman et al., 2001). It happens gradually, and far from every man notices it. By around seventy, roughly one in three men has a value that counts as low.
Important to know: a low number does not automatically mean symptoms, and symptoms do not automatically mean a low number. Fatigue, less interest in sex and a shorter fuse can go with low testosterone, but just as easily with stress, poor sleep or an underactive thyroid.

Why is muscle strength so important as you age?
Muscle strength is one of the strongest predictors of how your older years go. In the large PURE study, lower hand grip strength was a stronger predictor of early death than your systolic blood pressure (Leong et al., 2015). Muscles are not just for the mirror.
For men this is extra relevant, because muscle loss speeds up after fifty. Without pushback, you shed strength and mass on your own. That process has a name: sarcopenia. And it is more treatable than most men think.

What can you influence and what not?
Some factors sit largely in your own hands, others do not. It helps to keep those two apart, so you spend your energy on what you can change and accept the rest. The table below sets them side by side.

See this split as a compass, not a final verdict. The line between "yes" and "no" is sometimes grey. But the idea is clear: you have more influence than you think on the things that matter most.
How old is your body really?
Your calendar age does not say everything. Two men of sixty can differ biologically by years, depending on lifestyle, muscle strength and metabolism. That idea is called biological age, and it captures how fast your body changes better than your birth year.
There are all kinds of online "age tests", but far from all of them measure something reliable. What research does link to ageing are things like walking speed, muscle strength and certain patterns in your DNA. A single blood value is not a validated biological clock.

What should you watch for your heart?
Cardiovascular disease is the leading cause of death in men, and the risk rises with the years. The annoying part is that most risk factors are silent: you usually do not feel a high cholesterol or a raised blood pressure. That is exactly what makes measuring valuable.
The biggest dials are known: not smoking, a healthy weight, moving, and keeping an eye on cholesterol and blood sugar. Lifestyle does a lot here, and sometimes medication is needed. What a high cholesterol actually means is in cholesterol in men.
I mention this early on purpose, because the heart is one of the places where paying attention early pays off most. Recording your values once gives you something to compare against.
What about your bones?
Bone loss is often seen as a women's problem, but men get it too. And when a man breaks a bone because of osteoporosis, the risk of serious consequences is even higher than in women. In a large study, the excess mortality after a hip fracture was greater in men than in women (Bliuc et al., 2009).
Lower testosterone plays a part here, because the hormone helps keep your bones strong. Vitamin D, calcium and strength training matter too. The Dutch Health Council (Gezondheidsraad) advises men from seventy to take extra vitamin D.

Is preventive blood testing worthwhile?
Sometimes yes, sometimes no. A blood test without reason does not automatically bring health gains, and can even cause needless worry. In a large review, general health checks did not measurably reduce mortality (Krogsboll et al., 2019). Measuring for the sake of measuring makes little sense.

Which blood values give a baseline?
There is no fixed list that fits everyone. But a number of values give a broad view of how your body is doing, especially from forty onwards. Below you see which values roughly light up which area.
