Fatty liver sounds like something for heavy drinkers, but the most common form has nothing to do with alcohol. An estimated 1 in 4 adults in the Netherlands has a fatty liver, and most do not know it. It is strongly linked to belly fat, a high cholesterol and a sugar creeping just too high.

It is exactly that pattern you see a lot in middle-aged men. And because a fatty liver gives no symptoms for years, it often stays unnoticed until a blood test or scan shows something.

What is fatty liver?

Fatty liver means too much fat is stored in your liver cells. The variant that does not come from alcohol is often called NAFLD, or nowadays also metabolic fatty liver. A little fat is not immediately a disaster, but too much can over time cause inflammation and damage.

What are the causes in men?

Which signals does it give?

Honestly: usually none. A fatty liver in an early stage often runs without symptoms, and that is exactly why it stays under the radar so long. Sometimes there is vague fatigue or a pressing feeling in the upper right of the belly, but that is non-specific.

A fatty liver is often discovered by chance, via slightly raised liver enzymes on a blood test or via a scan done for another reason. That makes it tricky: you cannot rely on feeling it.

What can you do about it?

Here sits the gain, and the good news. Fatty liver in an early stage is often reversible, and the most important dial is lifestyle. Losing weight, even if it is only part of your belly fat, can often favourably affect the fat in your liver.

According to Thuisarts.nl, gradual weight loss, more movement and less sugar are the steps with the most effect for a fatty liver. No miracle cure, but a direction that works. And go easy on alcohol anyway, because that adds strain to your liver.

Is fatty liver dangerous?

Can you have fatty liver without drinking?

Yes, and it is even the most common form. The non-alcoholic variant is linked to belly fat, sugar and cholesterol, not to alcohol. So you can have a fatty liver while you barely drink.

Does a fatty liver go away again?

In an early stage often yes. Because your liver has a strong repair capacity, the fat can decrease if you lose weight and move more. The longer the fatty change has lasted, the more time recovery takes. That is why looking in time and tackling the causes is worthwhile, even without symptoms.
