Tamsulosin side effects in men: from dry orgasm to your eyes

The tamsulosin side effects men report most are dizziness on standing, a changed or dry ejaculation, a blocked nose and headache. A changed ejaculation affects 8 to 18 percent of users. The dizziness settles within a few weeks for many men. The dry ejaculation usually does not.

I find the patient leaflet unsatisfying here. "Abnormal ejaculation" is medically correct and tells you nothing about what happens in bed.

So below is what each side effect actually feels like, and whether it goes away.

Which tamsulosin side effects are most common?

Dizziness tops the list, mostly in the first days and mostly when getting up from bed or a chair. Then come a blocked nose, headache, and stomach or bowel complaints such as nausea or constipation. The leaflet notes that taking the capsule with some food often helps the stomach complaints.

That dizziness has a logical cause. Tamsulosin relaxes muscle tissue, and not only around your prostate. Your blood vessels contain the same kind of muscle.

Stand up quickly and your blood pressure dips briefly. That feels like a light head or dark spots. The Dutch Farmacotherapeutisch Kompas names this effect explicitly for this drug class.

Fatigue is reported by around 7 percent of users. That is enough to notice and too little to be sure the drug is the cause. Poor sleep from night-time urination does the same thing to your energy.

What is a dry ejaculation and does it go away?

With a dry ejaculation the orgasm arrives but little or no fluid comes with it. The semen heads toward your bladder instead of out, which is called retrograde ejaculation. It is not harmful, and for most men it resolves once you stop the drug.

This is where men go quiet, which is a shame. Something can be done about it.

How often it happens depends heavily on which alpha blocker you take. The 2014 systematic review and meta-analysis by Gacci and colleagues compared this between agents and found clear differences (PMID 24708055). Silodosin sits at the high end, alfuzosin at the low end.

Two things matter practically. Your urine can look cloudy after an orgasm, because the semen is in there. And if you still want children, this is worth raising with your doctor, because conception by the usual route is difficult without fluid.

Can tamsulosin cause weight gain?

Weight gain is not listed as a common tamsulosin side effect. What does happen: fluid retention and swelling of hands or feet are rarely reported, and on the scales that can look like a couple of kilos. Moving less because of fatigue often plays a part too.

The fact that people search for this does not mean the link is established.

The men looking this up are usually over sixty, often on more than one medication, and sometimes on a blood pressure drug as well. That last group does report fluid retention more often. We wrote about that in blood pressure medication side effects.

If you gain several kilos in a short time, with swollen lower legs or shortness of breath, that is one for your GP. Not because it must be the tamsulosin, but because that combination can have other causes.

What does tamsulosin do to your eyes?

Tamsulosin can make the iris floppy, which makes cataract surgery technically harder. This is called intraoperative floppy iris syndrome, or IFIS. So tell your eye surgeon you take it, even if you stopped months ago. Blurred vision and dry eyes are also reported at times.

Of every side effect on this list, this is the only one someone else has to act on.

Chang and Campbell described the link in 2005 in the Journal of Cataract and Refractive Surgery (PMID 15899440). In 2014 Chang and colleagues compared severity with tamsulosin against alfuzosin, and the risk profile differed between alpha blockers (PMID 24314842). A 2009 study in Ophthalmology examined the factors associated with IFIS (PMID 19243826).

Stopping shortly before surgery does not solve it, which is the awkward part. The effect on the iris can persist. Telling them is more useful than stopping.

Can tamsulosin keep you awake?

Insomnia is listed as a side effect, but with urinary symptoms the cause is often indirect. If you wake two or three times a night to urinate, your sleep is already broken before the drug does anything. If tamsulosin reduces that, sleep can improve instead.

That makes this a side effect you can only judge by keeping track.

A simple method: count for two weeks how often you get out of bed per night, and compare that with the two weeks before you started. If you notice you lie awake more often without needing to urinate, that is a different story from going more often. On the second situation we wrote frequent night urination in men.

Which side effects fade and which stay?

Roughly: side effects linked to blood pressure and adjustment often settle within a few weeks. Side effects linked to the drug's action on muscle at ejaculation usually stay as long as you take it. Here it is per complaint.

That last row is the only urgent one. Fast-onset swelling of face, lips or throat is not part of adjusting.

Two men aged 61 both take 0.4 mg tamsulosin. The first was dizzy for a week and notices nothing a month later. The second still has a dry ejaculation, exactly as the table predicts.

What I notice in that: the side effect that stays is almost never the one men were braced for.

What to do with this

Track for two weeks which complaint arrives when, and at what time you take the capsule. Take that list to your GP or pharmacist. A fixed dosing moment after the same meal is often the first thing that changes.

Say it too if the dry ejaculation bothers you. Alpha blockers differ in this respect, and you start that conversation or nobody does.
