Dapoxetine (Priligy): how it works, side effects and experiences

Dapoxetine is the first and only medicine developed specifically for premature ejaculation. It is a short-acting SSRI you take roughly 1 to 3 hours before sex, not every day. In research it lengthens time to ejaculation by around two to three times on average.

In the Netherlands almost everyone knows it under the brand name Priligy. Other SSRI antidepressants are sometimes used for this too, but outside their registered indication.

What strikes me: premature ejaculation is probably the most common sexual complaint in men, and at the same time the one written about worst. Usually it stops at breathing tips.

What does dapoxetine actually do?

Dapoxetine blocks the reuptake of serotonin in the nerve pathways that drive ejaculation. More serotonin there slows the ejaculatory reflex. The difference from regular antidepressants is speed: dapoxetine is absorbed quickly and leaves your body quickly too.

That short action is the entire point. You do not build up a daily level, so you avoid the side effects that come with continuous SSRI use.

Peak concentration sits roughly 1 to 2 hours after taking it. That is why the leaflet says to take it well before sex rather than just beforehand.

How well does it work according to research?

Several meta-analyses of randomised trials show that dapoxetine clearly lengthens time to ejaculation compared with placebo. In most studies the average gain is around two to three fold. Sexual satisfaction and sense of control also rose in the same studies.

An important nuance: averages hide large differences. Some men notice a lot, others barely anything.

The placebo effect in these studies is substantial too, which makes sense for a complaint where tension plays a big role. So the honest conclusion is: it often helps, it is not a switch.

Dapoxetine compared with the alternatives

There are broadly three medical routes for premature ejaculation, plus behavioural techniques. They differ in how fast they work and what they ask of you. Which route is appropriate is decided by your doctor together with you.

What side effects does dapoxetine have?

Nausea is the most reported side effect, followed by dizziness, headache and diarrhoea. They are dose dependent and usually gone within a day. Because you only take the medicine when needed, complaints stay limited to that occasion.

One side effect deserves separate attention: fainting. Dapoxetine can cause a sudden drop in blood pressure and heart rate, especially when standing up too fast.

That is why the leaflet says to drink enough water when taking it. It sounds like a detail, but it is the reason that warning exists.

Alcohol increases dizziness and the risk of fainting. That is an unfortunate combination, because men sometimes have a drink to relax about exactly this complaint.

Dapoxetine experiences: what do users report?

Experiences vary, and the common thread is that expectations shape the verdict. Satisfied men mostly mention that the tension lifts. Men who stop usually do so because of nausea, not because of lack of effect.

Take a man of 34 who tries it for the first time on a Friday evening, after two beers and without eating. He feels sick and dizzy, and the evening is over.

His conclusion is that the medicine is not for him. In reality he did three things wrong at once.

Other things users frequently report:

• The nausea fades after a few times
• The sense of control matters more than the clock
• Without a conversation with the partner, little changes
• It works better when the surrounding tension is addressed too

Can you combine dapoxetine with an ED medication?

Some men have both premature ejaculation and erection problems, and the two reinforce each other. Research into combining dapoxetine with a PDE5 inhibitor shows added value for some men. Whether that is sensible in your case is your doctor's assessment.

Both medicines can lower blood pressure, so the combination is not automatic. Combining them yourself without advice is exactly where it goes wrong.

When is blood testing useful?

For premature ejaculation on its own, blood testing is usually not the first thing your doctor considers. That changes when erection problems, low desire or fatigue come along too. Hormonal values then come into view.

What I would bring to the GP

Before you go, note roughly how long it takes, whether it has always been this way or only recently, and whether it happens with every partner and situation. Your doctor will ask those three questions anyway.

If it is recent, a cause is more often findable than when it has always been this way. That difference sets the whole direction of the conversation.
